What are the precautions for using elastic garments and elastic sleeves for scar treatment?

Pressure therapy is one of the most important methods for early treatment of scars. Pressure therapy should pay attention to the following points:

   (1) Start use as soon as possible: It is best to start after the sutures are removed or the wound is healed. The most effective stage of compression therapy is within six months to one year after the wound has healed. When using compression therapy, if there are wounds, blisters, etc. on the surface of the scar, you can temporarily stop using it, or put a sterile dressing on the wound to continue the pressure treatment.

(2) The compression force should be appropriate: it should not affect the blood supply of the limbs, and the patient can tolerate it to the limit. Usually the pressure can be controlled at 25~30mmHg. If the pressure is too low, it will not be able to treat it. If the pressure is too high High, it may cause venous return disorder, limb edema, and even more serious complications. During treatment, felt pads or gauze pads should be added to the depressions as pads to make the depressions evenly stressed. In the course of treatment, it was found that the pressure became less, and it was necessary to make adjustments in time. When performing compression therapy on the limbs and joints, it is necessary to pay great attention to the movement of the joints, and perform functional exercises appropriately to prevent the occurrence of joint stiffness.

(3) The treatment must have a certain length of time: due to the use of compression therapy, in addition to subjective discomfort, there are also problems that affect aesthetics. Therefore, a considerable number of patients cannot insist on using compression therapy, which affects The effect of treatment. Stopping compression therapy too early may cause rebound scar hyperplasia. Therefore, we should insist as much as possible, advocating continuous pressurization 24 hours a day, changing cushions and washing the skin at a time not to exceed 30 minutes, and compression treatment time not less than 3 months, generally more than half a year. If you feel too much pressure to bear, you can relax a little, but the time to stop it should not be too long. Generally, the compression therapy should be maintained for at least 3-6 months. After 8-12 months, most patients can stop the compression, but some patients Use it for 2 years.

(4) Pay attention to the fixed position of the scar: the front of the neck scar fixes the neck at the back of the head, the neck scar fixes the neck to the healthy side over flexion, the wrist, elbow, and knee scars can fix the joints in the extended position, the ankle joint Fix the joints in a neutral position with scars, and fix the metacarpophalangeal joints at 90° flexion with the scars on the back of the hand, the thumbs at the opposite palm position, and the fingers at the straight position.
